Section Specialty Day: Endoscopic Spine Surgery
Spine surgery is advancing towards ambulatory centers while same day surgical centers are gaining popularity among spine surgeons. As the number of complex fusion surgeries increase, especially in elderly patients with multiple comorbidities, more than ever there is a need to develop new minimally invasive revision strategies.
The endoscope has enabled spine surgeons to perform several surgeries, which previously required large incisions, through small keyhole incisions. Initial indications were routine discectomy, recurrent disc herniation, far lateral discectomy surgeries to interbody fusion through small incisions. The indications are continuously expanding to include several aspects of spine surgeries such as foraminal decompression at the level of previous fusion for optimal decompression, decompression of adjacent level, and repair and replacement of interbodies.
Patients with mild to moderate spinal deformity are often compensated and have tolerable levels of back pain. However, unilateral radicular pain resulting from foraminal stenosis can be debilitating. In select cases, an endoscopic discectomy or foraminotomy enables the surgeon to decompress the symptomatic foramen with preservation of essential biomechanical structures, delaying the need for a major deformity correction surgery.

We have been introduced to Lam Thi Hue by Viet Duc Hospital, the local hospital and Wooridul Hospital’s partner. While with the spinal deformity, Lam Thi Hue was troubled with lying flat and was in intense pain, the family circumstances only allowed a monthly income of approximately 100 dollars and getting treatment was hardly thinkable. Scoliosis is a serious disease that does not only bring about problems in appearance but it can also translocate or press organs, and lead to dysfunction. The sad situation has made us Wooridul Hospital to support the sufferer.
We checked the medical record and X-ray result for planning and saw that a surgery would be needed from the 4th Thoracic Vertebra to the 1st lumbar vertebra. The surgery was to be a tough one for a 14 year old girl but with positive heart, we welcome her and her family. In the early morning on the 28th of June, we came to meet Lam Thi Hue and her mother in a very tired and nervous look. We could understand that all the way from a small town in Vietnam to a whole another country, South Korea, with the hope to be treated, the mother and daughter must have been through numberless tensed moments. From their hands firmly held together, we could feel their fear and also their desperate courage. We made up our mind to do our very best.
Arriving to the hospital, Lam Thi Hue went through a couple of examinations for diagnosis and we came up with the final surgical operation plan and then the preparations. The examination for the surgery preparations was nice and quick. We were worried that both the patient and parent would be tired since all steps were proceeded in the date of entry day due to the visa issues, but they looked relaxed during the doctor’s explanation and the examinations. They looked as if they realized about the treatment.
Before the surgery, we, by the curative procedure of Wooridul Hospital, reconfirmed the plan to find the best treatment and to maximize the rate of success, based on the knowledge and experience of our neurosurgeons, orthopedists, physicians, specialists in rehabilitation, radiologists, and doctors in general surgery.
Lam Thi Hue headed to the operating room at 8.30 in the morning. She endured the tough 5 hour long surgery and from the next day, she started walking and showed fast recovery. The pain from the surgical site and the influenza must have been hard but Lam Thi Hue was bright enough to comfort her worrying mother and carried on with her brave life of hospitalization.
However, the Korean and European meals could not replace the local Vietnamese meals and so we provided the Vietnamese meals from outside the hospital to be a help in her fast recovery.
Lam Thi Hue could enjoy our surprise present of visiting Coex and the S.M. Town. And in the dashing figure posing like celebrities to take photos, and in the straight and upright position in the aquarium, she did not look different from any other teenager girls.
On the discharge date, giving a hug to the doctor and saying “Gam-sa-ham-ni-da!” repeatedly, Lam Thi Hue expressed with her whole body, her feelings about the kindness she experienced in Korea, and said that she would really love to come back to Korea. But that all changed when patients kept complaining about the pain, for specialists needed to destruct back muscles and destabilize the spine itself to see the herniated disc, Bae said. “We think differently. With MISS, we focus on treating the pathology with different clinical presentations and interventions,” he added.
With almost 16 years of training and 20 published articles under his medical belt, Bae is a recognized spine specialist with expertise in cervical, thoracic and lumbar spine surgeries. He received the “Best Paper Award” at the 2016 Society for Minimally Invasive Spine Surgery (SMISS) Annual Forum, the only major meeting focused entirely on MISS.
“We believe our technique should be taught around the world, through which we develop it further every year,” Bae said. “I perform about 40-50 surgeries a month. We have 10 spine specialists here, meaning we can treat around 500 patients at most. If we teach other doctors, we can share our knowledge and treat more patients through them."
Wooridul’s International Training Course provides not only the MISS Course but also a fellowship course and an operation observation session as well.
Bae said, “We can only offer limited seats; seven participants on average for a course. We don’t want a big audience like a lecture, for we need to discuss in depth to be efficient for the convenience of patients.”
The fellowship course provides two or three foreign surgeons with a year of intensive training in diagnosis and treatment of Minimally Invasive Spine Surgery & Techniques (MISST). This includes observations in operating rooms, participation in Friday teleconferences with Wooridul’s hospital network, clinics research projects, and writing their papers and journals.
“Wooridul received many positive feedbacks from foreign doctors once they returned to their country,” the spine specialist said. “We had beginning surgeons start their endoscopy clinics. By the app called Whatsapp, we can discuss, review and follow-up their developed skills and to help their patients.”
Foreign doctors are not the only ones impressed with Wooridul’s medical techniques and services; over 12,000 international patients have visited the global spine medical hub, attracted by the fact that months or even years of back pain could be lifted in a simple 30-minute surgery and could walk home the very same day.
Patients with severe diseases require 10 hours of surgery, but with two to three specialized consultant doctors for assistance, the operating time decreases by half.
“Arab patients tend to stay as long as possible; between one or two months for rehabilitation in our physical therapy clinic,” Bae said. “Other patients such as Chinese, Japanese and Russians wish to go back to their countries as quickly as possible, so they stay in Korea for a week.”
Although Wooridul’s programs are small and elusive, Bae confirmed plans for a two-day cadaver course in their wet-lab next year and welcomed Korean and foreign doctors alike.

Jun Seok Bae-Neurosurgeon, Medical director of Gangnam Wooridul Hospital, returned back to South Korea on July 14th with tanned black face after completing his schedule of 5 days from Maharashtra in western India.
Medical Director, Jun Seok Bae had tough schedule with daily 2~3 hours of movement to consult and caring patients as much 900 patients a day in unfamiliar named wilds that takes 10 hours to get there by car from Mumbai (capital city of the Maharashtra state). “Local circumstances were very poor to get enough medical care and most of them were first time to getting the medical benefits. Women were marring at their young age and does heavy work. There were no one without anemia, with bad nutritional status and back pain and knee pain, severe systemic pain…Also most patients needed urgent treatment.
We prepared necessary equipment and medicines as much as possible with supports of Indian NGO organization, the Spring of Desert and South Korea Love community society, Wooridul Hospital Social Responsibility support program, but the number of patients who wanted to have treatment were over than our expectation. That’s why we returned the people who were waiting until the end. It was really regretful and felt so sorry for them.
However we delivered the hopeand all staffs who participated in Medical healthcare Volunteer, everyone have done their best. Although medicine and injections were all, but patients shed tears of gratitude. It was precious time that realized me sense of duty as a doctor once more.”
